Arizona Assisted Living Facilities Government Services

The Arizona Department of Health Services is responsible for all inspections of Arizona assisted living facilities. These inspections are performed on an annual basis and records are maintained at the Department of Health Services. You can contact them to request copies of an inspection report at 602-364-2639. You may also use this number for checking on complaints for a specific assisted living facility and for filing a complaint. Be prepared to share specifics about an incident and the personnel involved when reporting a complaint.
