This is a real chamber of horrors. No competent supervision. No continuity of care. Accident alley. No care plans. Agency vs. in-house staff. Suspicious characters roaming halls and entering resident rooms. Food quality-NONE. Residents dehydrated. Incompetent management. Food prep areas filthy--shoes stick to floor. NO WOUND NURSE. NO COMPLIANCE WITH PHYSICIAN ORDERS. MEDICATION ERRORS. NUMEROUS INJURIES, FALLS, ACCIDENTS. AVOID THIS HELL HOLE AT ALL COST. YOUR LOVED ONE WILL SUFFER AND DIE PREMATURELY.

High ratio of agency vs. in house staff. No continuity of care. Poor Security with outside, suspicious persons roaming the halls and entering resident rooms. Kitchen food prep areas are filthy. Food is 4th rate, with no consideration for special diets, i.e., mechanical, soft, etc. Numerous residents suffer from dehydration, accidents-falls, medication errors, failure to implement physician orders. This place is ghastly, filthy, and dangerous. LITTLE OR NO PROFESSIONAL SUPERVISION. INCOMPETENT MANAGEMENT
