Beacon Hill Rehabilitation

My mother was recently a patient here, nursing staff was not in tune to medical changes occurring with my mother and often took myself or my sister to point these out and request the nursing staff contact the physician. My mother had a low blood pressure on 4/25 and was very fatigued this was reported to the day nurse by CNA. Day nurse continued to do his medication pass, I phoned the physician at 1640 she called the facility at 1700 and gave the day nurse verbal orders for IV fluids and he came into my mother's room and let us both know that he would not be starting these IV fluids until he completed his medication pass. I left and returned to the facility at 2030 and my mother still had no fluids or IV placed. Finally between 2130-2200 an IV was placed and fluids infusing. I phoned the next am at 0825 to check on my mother and the same day nurse answered the phone stating that my mother was " not doing so good, she has a fever of 101, is lethargic, and is refusing to go to the ER." I immediately went to the facility asked said nurse what time he was aware my mother had a fever and he said "7." I asked why family had not been notified of the change in condition and he stated "I gave her Tylenol and called the doctor right before you called." I asked said nurse if he knew how dangerous it was to sit on a fever for an hour and a half in a frail elderly person, he then stated "I'm not going to be talked to like an idiot, and I'm thru with this you can talk to my boss." I then assessed my mother she was found oxygen saturation's of 85 percent on 2 liters of oxygen, BP of 70/palp, HR 112, respiratory rate of 20. She was lethargic and difficult to arouse. I called 911 as another facility nurse told me I didn't need to do all this as they would and I instructed her that's the problem they are not. Once at the hospital my mother's temperature was 102.4. BP 78/40 HR 110. My mother was sent over with the IV placed by the SNF which was not capped and pointed out to me by the ER nurse. My mother was in severe sepsis and ultimately died as a result of complications related to this. My mother was very frail, elderly, and compromised which I completely understand, however when you entrust your loved one to professionals you expect them to be just that professional and report changes to physician at once, not finish a medication pass. You also expect their assessments to be thorough and family who are medically incline should not have to do their job. You expect if they receive physician orders they act upon these, not 5 hours later because family is there prodding staff. If you're looking for mediocre care for your loved one you should definitely take them to Beacon Hill Rehab. The therapy staff was great and were much more in tune to my mother's medical conditions than the nursing staff. I feel there was definitely negligence on the part of the day shift nurse both 4/25 and 4/26, certainly he would not want his mother treated this way. Again I do not recommend this facility.
