Cottages of Clayton

My mom lived here for a year. In that year her physician consulted with me 3 times about her
failure to gain wait. I spoke to the nurse, the Executive Director (Ken) and wote a letter to the
owner! My mom said that if she wasn't on time to meals there was nothing left and she
couldn't eat. I didn't believe this until I witnessed it for myself! I was paying for meals and she
was not eating! On another occasion they gave her medication for her blood pressure twice,
causing her to nearly die. While waiting on an apt to come open down the road, which was
two weeks it cost my mom her life. She evidentially wanted to call me for something and they
unplugged her phone and told her it didn't work according to her neighbor lady. She couldn't
see the buttons and needed help dialing. From what I understand she kept asking the aid over
and over. She wasn't found until I visited the next day and found her in her apartment dead.
She was still in her clothes from the day before when I saw her. This means no one checked
on her when she missed dinner, no one gave her night medication or they would have found
her. Her neighbor said the last she saw her was 4 o'clock when she wanted to call me. Don't
let them kill your loved one! Its a mistake I will never, ever get over!

Med errors,unconcerned owner,administrators & nurses. Injuries to
residents,residents apparently left unattended, med errors, Dr. orders
not followed. Clean attractive facilities out side residents rooms. very
nice aids... My personal advice is I would not send my dog to this
place I think a shelter would do a better job. I have reported this
facility to the state of Ohio 3 times for med errors,unclean rooms 2
separate injures to my Mother(Black eye and broken wrist).

This place has had several complaints with the state for how they
treat the people that live there. Medications are not given right, accu-
checks are not done right and patients living there have unexplained
injures that the staff does not know how they happened. Patients are
not taken care of properly. The patients rooms are not cleaned
good. There have been times I have gone to visit my family that live
here and have found food and dirt in and around the bed area. Pills
on the floor. There has been soiled clothing around the room and
stool on the toilet and floor. We are moving our family far away from
here. Ken is the director who "Takes full responsibility" but he does
not fix the problems. He will tell you what you want to hear then do
nothing to fix the many problems!
