Resident & family reviews
Father was admitted from ER to hospital. Medication list reviewed by nurse after transfer to hospital was a list for the WRONG PATIENT. Luckily patient's family members recognized the error; otherwise wrong medication may have been administered! At discharge, family realized that one medication was NOT administered during the 5-6 days of confinement. No explanation forthcoming.
