Carbondale Nursing and Rehabilitation Center is dually certified in Medicare and Medical Assistance. Our nursing department is staffed by registered nurses, licensed practical nurses and certified nursing assistants who provide 24-hour care. All care and services are under the supervision of a Medical Director. Our dietary department is under the direction of a Registered Dietitian.
