My family trusted Oak Leaf to assist my grandfather who was not
accounted for more than 14 hours while he lie injured in his room from
a fall. He was dead on arrival at a local hospital. Referral made to PA
Department of Welfare. I would not trust having my loved ones at
this incompetent facility.
