Services
At Angel Hands, we are very proud of the medically directed, team oriented care we provide to each
of our patients. We work closely with the other professionals involved in your case to design the
best plan of care and support possible for each patient and their loved one. Teams will vary
depending on your specific needs, but typically include:
Physicians, Nurses, Qualified Hospice Aides, Social Workers, Chaplains and Spiritual Counselors,
Bereavement Coordinators and Volunteers
Angel Hands also provides diagnosis related medications, medical equipment and supplies necessary to
promote the highest degree of comfort at home or in other hospice settings.
The majority of hospice care is provided in the patient's home, with family and friends acting as
caregivers. If there is no residence, the agency can provide care in nursing homes or other types of
residential facilities. If a patient requires inpatient care in order to address severe symptoms or
to provide respite for a caregiver, we have relationships with hospitals and nursing home facilities
throughout our service area, allowing patients to be served in their own communities.
Not all problems occur between the hours of 8 and 5. We have a triage team available 24/7. When you
call in the evenings or on weekends, we will always be available, even if the need is not medical in
nature. If an evening or weekend visit is needed, you can count on us to be there for you.
